Introduction
Thank you for taking the time to complete the application for community partners. By completing this application you will be able refer children to the program as well as other functions related to the program. To the left you will see all of the steps of the online application. As you complete each step it will be checked off. Click on the green arrows at the bottom of the page to change steps. You must complete this application at this time, it is not possible to return at a later time.

Login Information
This section contains all of the information to establish your account with www.gvsupals.com. This information will allow you to check the progress of your application, update your references as needed and other critical functions during the application process. Pals Student Mentors uses e-mail as the primary form of communication for all participants so please use a e-mail address which you check frequently.

Leader Information
This step of the application requires basic information about the leader. All fields are required and the first and last name box are limited to 20 characters each.

Organization Information
Please enter the name and description of your organization. Several different types of content may be added to the description in order to allow for a rich user experience including images and links. If you have specific questions about these features please e-mail us at administrator@gvsupals.com.

Contact Information
Please add as much information as possible to identify your organization.

Phone Numbers
A minimum of one phone number is required. The number must be in the following format is 616-331-5000. You may add as many phone numbers as you would like. The description field is optional and may be used to add extra information regarding the numbers (best time to call, etc.)

Addresses
A minimum of one address is required, which should be a local address. The address is geocoded to verify that it is in the correct format. You are asked to attempt to verify an address three times prior to submitting an unverified address. You may add as many addresses as you would like. The description field is optional and may be used to add extra information regarding the address.

Feedback and Submit
This is the final step of the application. If you have any additional information, questions, or concerns please include them in the comments box. The comments box is limited to 400 characters. Once you are satisfied with your application please click the Finish button.

Application Restricted
